Reconciliation Forms
Below you will find two links to statement reconciliation forms. Which one you choose to use will be determined by the process you use to reconcile your account(s).
A. Manual reconciliation (paper and pencil)
Option 1 – if you want to save a copy of the form for future use
- Click on the PDF form to display it on your computer.

Locate the Save icon in Adobe Acrobat Reader and click it. One
of the two toolbars shown below will come up on your screen
depending on what version of Adobe Acrobat Reader you have.
- Your form will be saved to your home computer.
- Go back into the form and locate the printer icon in Adobe Reader.
- Click this and the form will print on your home printer.
Option 2 - if you just want to print the form:
- Click on the PDF form to display it on your computer.

Locate the Print icon on the Adobe Acrobat Reader toolbar and
click it.
- Your form will be printed on your home printer but not saved to your computer.
B. Electronic reconciliation (requires that Microsoft Excel be installed on your computer):

Click the Excel form. When the dialog box comes up asking you to
save or run the form, click save.
- Choose a location on your computer to save the form.
- Be sure to make a record of the location so you can find it later.
- Start Microsoft Excel.
- Open the form (browse to the location on your computer where you saved it)
- Proceed as you would with any other spreadsheet.
- Save the completed form.
Please note, you may print out or save as many copies of either of these two forms as you would like. If you don’t have a printer at home, please feel free to ask us to print this out for you next time you visit us.
